Service Coordinator - Remote

Overview

Venture Forthe is one of the largest and fastest-growing Licensed Home Care Agencies in Western NY! We are looking to add a Service Coordinator to our team to provide services in Cortland, Madison, Tompkins, Tiaoga, Broome, and Cayuga counties.

In Short

• Assist prospective participants to become waiver participants.
• Coordinate and monitor the provision of all services in the Service Plan.
• Work independently and create a flexible schedule.
• Manage a caseload of 20 clients.
• Advocate for participants to ensure they receive appropriate services.
• Conduct assessments and reassessments of participants' levels of care.
• Develop and implement Service Plans reflecting participants' goals.
• Provide training and resources for the team.
• Maintain quality assurance for services provided.

Requirements

• Valid NYS Driver’s license and ability to travel.
• Experience with TBI and/or NHTD Waiver service population.
• One year of experience in case management/service coordination.
• Qualifications include various licenses or degrees in social work, psychology, or related fields.
• Ability to manage community resources and build Service Plans.
• Experience in educating others on Waiver rules.
• Supervision by qualified Service Coordinator required for agencies with multiple coordinators.
