Overview

UCP Oregon is seeking Personal Agents (Caseload Managers) to work with adults experiencing disabilities, helping them navigate support services and achieve their goals.

In Short

• Work with 36 to 40 adults on your caseload.
• Assist customers in navigating support services.
• Use person-centered planning techniques.
• Secure resources in employment, housing, and crisis support.
• Advocate for the people you support.
• Manage intensive paperwork and documentation.
• Monitor the quality of services provided.
• Work in a hybrid remote and office-based model.

Requirements
