Overview

The Personal Agent role at UCP Oregon involves managing a caseload of adults with disabilities, helping them navigate support services and advocating for their needs.

In Short

• Work with 36 to 40 adults on a caseload.
• Assist customers in navigating support services.
• Use person-centered planning techniques to assess needs.
• Secure resources in employment, housing, and crisis support.
• Address health and safety needs of customers.
• Manage intensive paperwork including plan development and billing.
• Monitor quality of services provided.
• Work in a hybrid model (2 days in office, 3 days remote).
