Overview

The Transplant Liaison plays a vital role in guiding End-Stage Kidney Disease (ESKD) and waitlisted patients through the transplant process, ensuring seamless support from referral to transplant completion.

In Short

• Act as a key advocate for ESKD and waitlisted patients.
• Facilitate access to transplantation through collaboration with healthcare teams.
• Support transplant evaluations and optimize wait listing processes.

• Regular travel to meet patients in NJ and PA.
• Work from home for remaining days.
• Minimum of 3+ years of experience in transplant coordination or nephrology required.
• Strong problem-solving and communication skills necessary.

Requirements

• Medical Assistant (MA) or Care Coordination experience required.
• Bachelor’s degree in Health Care Administration or Biology preferred.
• Experience with ESKD and transplant patients in a clinical setting.
• Knowledge of transplant evaluations and living donor programs.
• Ability to navigate healthcare systems and community resources.
• Experience with electronic health records (EHRs).
• Strong patient advocacy skills.
• Ability to work cross-functionally with all teams.
