The Care Coordinator I role involves supporting patients with chronic conditions through phone-based interactions, focusing on care coordination and documentation.

In Short

• Support patients with chronic conditions through phone calls.
• Make 30–50 outbound calls per day.
• Conduct monthly Chronic Care Management (CCM) phone calls.
• Educate patients on care plans and chronic conditions.
• Coordinate care with various healthcare providers.
• Document all patient interactions accurately.
• Maintain HIPAA compliance.
• Work full-time, 40 hours per week, in a fixed schedule.
• Comfortable with high call volume in a remote environment.
• Requires a private home office with high-speed internet.

Requirements

• 1-3 years of experience as a CMA/RMA, LPN, or RN.
• Active certification or multistate license for LPN/RN.
• Strong verbal and written communication skills.
• Experience with EMR/EHR systems.
• Ability to work independently in a remote setting.
• Geriatric patient experience is preferred.
