In Short

• Assess and Support Patients: Assist patients with overcoming physical, cognitive, and emotional barriers.
• In-Home Assessments: Conduct comprehensive in-home assessments.
• Collaborative Care Plans: Develop individualized care plans.
• Interdisciplinary Collaboration: Work closely with other healthcare professionals.
• Adaptive Equipment & Safety: Assess and train clients on adaptive equipment.
• Ongoing Monitoring: Reassess patients’ progress and adjust care plans.
• Documentation: Maintain accurate documentation of patient care.
• Continuous Education: Participate in ongoing professional development.

Requirements

• Education: Completion of a recognized Occupational Therapy Program.
• Experience: 3+ years of experience preferred.
• Clinical Skills: Experience with assessments and treatment planning.
• Communication: Strong communication and interpersonal skills.
• Technology: Proficiency in computer software and virtual communication platforms.
• Independence: Ability to work autonomously and within a team.
• Driver’s License: Valid Ontario driver’s license required.
• Compassion: A caring, enthusiastic attitude toward clients.
