In Short

• Assist patients with overcoming physical, cognitive, and emotional barriers.
• Conduct comprehensive in-home assessments.
• Develop individualized care plans with client input.
• Work closely with interdisciplinary teams.
• Assess and train clients on adaptive equipment.
• Reassess patients’ progress and adjust care plans.
• Maintain accurate documentation of patient care.
• Participate in ongoing professional development.
• Demonstrate compassionate behavior in all interactions.
• Utilize both in-person and virtual assessments.

Requirements

• Completion of a recognized Occupational Therapy Program.
• 3+ years of experience preferred in community settings.
• Experience with assessments and treatment planning.
• Strong communication and interpersonal skills.
• Proficiency in computer software and virtual communication platforms.
• Ability to work autonomously and in a team.
• Valid Ontario driver’s license and reliable vehicle.
• A caring and enthusiastic attitude.
