In Short

• Assess and support patients to maximize their independence.
• Conduct comprehensive in-home assessments.
• Develop individualized care plans with input from clients and families.
• Collaborate with healthcare professionals for optimal client outcomes.
• Assess and train clients on adaptive equipment and safety.
• Reassess patients’ progress and adjust care plans accordingly.
• Maintain accurate documentation of assessments and treatment plans.
• Participate in ongoing professional development and training.
• Demonstrate compassionate and respectful behavior in all interactions.

Requirements

• Completion of a recognized Occupational Therapy Program.
• Licensed with the College of Occupational Therapists of Ontario (COTO).
• 3+ years of experience preferred, ideally in community settings.
• Experience with assessments and treatment planning.
• Valid Ontario driver’s license and reliable vehicle.
