Overview

The Ambulatory Clinical Pharmacist (ACP) provides integrated support to patients and physicians by improving the quality of patient care through personalized relationships and medication management.

In Short

• Provide medication therapy management including medication reconciliation and adherence counseling.
• Collaborate with interdisciplinary care teams to develop individualized care plans.

Requirements

• Doctor of Pharmacy (PharmD) and licensed pharmacist in state of employment.
• 3+ years of experience as a clinical pharmacist; PGY1/PGY2 residency preferred.
• Strong verbal communication skills and knowledge of pharmacology.
• Ability to analyze information and exercise good judgment.
• Organized, high-integrity, and attention to detail.
• Team player with strong personal and communication skills.
