In Short

• Manage a caseload of individuals with Developmental Disabilities.
• Support individuals on caseload with English-Spanish interpretation.
• Prepare written reports and Personal Plans in both English and Spanish.
• Provide advocacy support and education regarding service options.
• Complete required records and DHS mandated forms.
• Coordinate home and community visits.
• Regular Monday through Friday schedule.
• No on-call requirement and no weekend work schedule.
• Hybrid Telework opportunities offered.
• Potential for loan forgiveness through the Public Service Loan Forgiveness Program.

Requirements

• Bachelor’s Degree in Human Services or a related field.
• Minimum one-year of experience working with individuals with developmental disabilities.
• Proficient in English and Spanish.
• Eligible for QIDP status.
• Valid Driver’s License and reliable transportation.
