Requirements

• 4 to 6 years of relevant experience.
• Bachelor's in Nursing or Master's in Social Work preferred.
• Active Licensed Clinical Social Worker or Licensed Professional Nurse.
• 5 years of clinical practice experience in Palliative or Renal care.
• Intermediate computer skills in MS Office.
• Discretion and judgment in handling confidential information.
• Certification in Case Management or Nephrology preferred.
• Willingness to travel up to 25%.
