Clinical Care Manager (RN) - Remote

Overview

The Clinical Care Manager (RN) will provide remote health coaching to members at risk for chronic kidney disease or those who have chronic kidney disease, working to enhance their understanding and management of their health conditions.

• Full-time employment with a salary range of $78,000 - $82,000.

Requirements

• Active RN license in good standing.
• 5+ years of RN experience.
• Flexibility to work PST hours.
• Experience in developing care plans for chronic conditions preferred.
• BSN and CDE or CDCES certifications are highly preferred.
• Bilingual in Spanish is a significant advantage.
• Experience working remotely as a nurse is preferred.
• Ability to travel occasionally for corporate events.
• Proficient with technology and communication tools.
• Strong written and oral communication skills.
