Overview

As a Clinical Health Coach at Renalogic, you will provide health coaching support to members with chronic kidney disease, while also managing projects and analyzing data to improve clinical practices.

In Short

• Deliver health coaching support via phone to members with chronic kidney disease.
• Monitor and implement quality improvement initiatives.
• Conduct clinical audits and collaborate with Clinical Care Managers.
• Provide training and support to newly hired staff.
• Collect and analyze data for project management.
• Lead clinical objectives and serve as the subject matter expert.
• Manage project scope and objectives for successful execution.
• Perform comprehensive clinical reviews of high-cost claimants.
• Participate in video meetings and engage with team members.
• Be adaptable and creative in problem-solving.

Requirements

• Must have an RN license in good standing.
• At least five years of RN experience in a related role.
• Experience in clinical quality or project management is preferred.
• Willingness to work evenings and weekends as needed.
• Ability to travel for corporate events and conferences.
• Strong technological skills and proficiency in Microsoft products.
• Excellent written and oral communication skills.
• Self-starter with a creative mindset.
• Ability to manage competing deadlines.
• Reliable internet and workspace for remote work.
