Overview

The Waivered Services Case Manager assists clients on a waiver to access needed waiver and State plan services, along with medical, social, educational, and other necessary services, ensuring they can live in the least restrictive setting possible.

In Short

Requirements

  • Experience in case management or related field.
  • Knowledge of waiver services and state plans.
  • Strong communication and advocacy skills.
  • Ability to develop care plans based on assessments.
  • Familiarity with crisis intervention techniques.
  • Proficient in documentation and record-keeping.
  • Ability to work independently and as part of a team.
  • Understanding of client confidentiality and ethics.
  • Organizational skills to manage multiple cases.
  • Willingness to travel to client locations as needed.
