Overview

The Bilingual Case Manager will play a critical role in engaging with individuals living with serious mental illness and substance use disorders, primarily working remotely with occasional on-site visits.

In Short

  • Maintain a caseload of individuals using evidence-based approaches.
  • Support members with social support navigation.
  • Conduct periodic telephonic and SMS outreach.
  • Work with members to identify health and wellness goals.
  • Collaborate with care management teams.
  • Meet monthly encounter metrics for compliance.
  • Assist individuals in securing community support connections.
  • Coordinate physical care management appointments.
  • Utilize online platforms for collaboration.

Requirements

  • 1+ years of general work experience, preferably in case management.
  • Bilingual in English and Spanish.
  • Strong understanding of cultural fluency.
  • High degree of empathy and collaborative skills.
  • Strong technical skills with CRM databases and office software.
  • Valid driver’s license and auto liability insurance.
