Registered Nurse, Integrated Care Manager - Remote

Overview

This position coordinates care for Oak Street Health members, working collaboratively with interdisciplinary care teams while functioning with professional autonomy under supervision.

• Manage an assigned caseload of members.

Requirements

• Current RN License in good standing.
• Minimum of three years clinical experience.
• Care Coordination / Case Management training.
• Knowledge of community resources.
• Demonstrated customer-focused interpersonal skills.
• Outstanding verbal and written communication skills.
• Excellent organizational skills.
• Independent problem-solving skills.
• Experience working with diverse communities.
• Familiarity with Electronic Medical Records platforms preferred.
