Registered Nurse Care Manager - Remote

Overview

The Registered Nurse Care Manager will work with members and providers to coordinate care for patients with high needs, ensuring quality and cost-effective outcomes.

In Short

• Conduct comprehensive assessments for case management eligibility.
• Develop and implement individualized care plans.
• Perform face-to-face or home visits as needed.
• Monitor and evaluate care plans for effectiveness.
• Maintain a regular outreach schedule for case management.
• Promote integration of services including behavioral health.
• Facilitate interdisciplinary care team meetings.
• Use motivational interviewing techniques to support members.
• Assess barriers to care and provide necessary coordination.
• Local travel (25-40%) required for member visits.

Requirements

• Graduate from an Accredited School of Nursing.
• 1-3 years of experience in case management or similar fields.
• Active RN license in good standing.
• Valid driver's license and reliable transportation.
• Bachelor's Degree in Nursing preferred.
• 3-5 years of experience preferred.
• Certified Case Manager (CCM) preferred.
