Overview

The Scheduler for Home Health & Hospice is responsible for scheduling clients' services while ensuring clinician continuity and effective communication among all parties involved.

In Short

• Full-time remote position.
• Responsible for scheduling home health and hospice services.
• Requires effective communication with patients, clinicians, and managers.
• Must exercise judgment in scheduling and rescheduling patients.
• Monday to Friday schedule with one weekend a month.
• High School diploma required; Associate's Degree preferred.
• 2-3 years of home care or hospice experience preferred.
• Experience with Microsoft Office and remote technology required.
• Valid Driver’s license and reliable transportation needed.

Requirements

• High School diploma or equivalent; Associate's Degree preferred.
• 2-4 years of industry experience.
• 2-3 years of home care or hospice experience preferred.
• Proficient in Microsoft Office applications.
• Experience with Trinity Health At Home software preferred.
• Strong multi-tasking skills on multiple computer systems.
• Reliable internet connection for remote work.
• Ability to work flexible hours.
• Valid Driver’s license and transportation required.
• Strong communication skills.
