Overview

The Pharmacy Technician, Population Health supports the Trinity Health Alliance of Michigan pharmacy team to enhance workflow for clinical pharmacy services, focusing on medication therapy management and patient outreach.

• Supports clinical pharmacy services throughout the network.
• Conducts patient outreach and scheduling for medication management.
• Provides telephonic patient care.
• Maintains and tracks data for clinical pharmacy metrics.

Requirements

• Michigan Board of Pharmacy Technician registration/license required.
• Certified Pharmacy Technician (CPhT) preferred.
• Three years of pharmacy technician practice required.
• Experience with patient outreach programs preferred.
• Strong verbal and written communication skills.
• Ability to work independently and collaboratively.
• Good knowledge of managed care environment preferred.
• Computer competency required.
• Ability to handle frequent interruptions and meet deadlines.
• Physical ability to use a computer keyboard and sit/stand for long periods.
