Case Manager II – EHR Team (Emerging High Risk) - Remote

Overview

The Case Manager II will support Medicaid members across Virginia, providing remote case management with fieldwork for home assessments.

Requirements

• 1–3 years of experience in case management or a related field.
• Strong computer and documentation skills.
• Valid driver’s license and reliable transportation.
• Accredited LVN/LPN program or relevant degree required.
• Active and unrestricted licensure in good standing (if applicable).
