Overview

The Case Manager will work with members and healthcare teams to coordinate and facilitate integrated care for individuals with high needs, focusing on behavioral health and long-term care.

In Short

  • Support New York HARP Adult Behavioral Health program.
  • Requires New York LMSW, LCSW, or LMHC licensure.
  • 1-3 years of case management experience required.
  • Conduct clinical assessments and develop case management plans.
  • Monitor care plans and provide ongoing member support.
  • Facilitate interdisciplinary care team meetings.
  • Local travel of up to 40% may be required.
  • Work schedule Monday - Friday, 8:30 AM to 5:00 PM EST.
  • Utilize motivational interviewing techniques.
  • Promote integration of services for members.

Requirements

  • Completion of an accredited LVN or LPN Program or a Bachelor's/Master's Degree in a relevant field.
  • Active and unrestricted licensure in New York.
  • 1-3 years of experience in case management or related settings.
  • Valid driver’s license with a good driving record.
  • 3-5 years of preferred experience in the same fields.
  • Knowledge of behavioral health services and community resources.
