Join Mindoula Health as a Care Manager in the Teen Suicide Prevention Program, where you'll provide vital support to individuals and families affected by suicide ideation and attempts.

In Short

• Full-time position with working hours from 10:00 AM to 6:30 PM Pacific Time.
• Requires availability for 20% of shifts on weekends and evenings.
• Assist with outreach, engagement, and continuous support for members.
• Develop and coordinate community resources for medical and social needs.
• Perform crisis interventions and ensure safety through tailored safety plans.
• Collaborate with members to assess care options and develop service plans.
• Educate members about mental health and available resources.
• Assist in scheduling appointments and follow-ups.
• Maintain accurate records of case management activities.
• Compassionate and detail-oriented approach required.

Requirements

• Bachelor's degree in psychology, social work, or related field required.
• Master's degree preferred.
• At least 2 years of experience in a related role.
• Crisis intervention training or certification required.
• Knowledge of local resources and social determinants of health necessary.
