Overview

This role involves receiving and processing referrals for home care and hospice patients, ensuring accurate data entry and communication with healthcare providers.

In Short

  • Receive referrals for Home Health/Hospice Services.
  • Review clinical documentation for patient admission.
  • Communicate with hospital Case Management.
  • Assist patients and their families.
  • Ensure integrity of patient data.
  • Prepare materials for Case Coordinators.
  • Perform other related duties as assigned.

Requirements

  • Technical Degree or Diploma in Nursing.
  • 1-3 years of administrative or customer service experience in healthcare.
  • Excellent customer service skills.
  • Knowledge of third-party payers.
  • Ability to manage multiple tasks and prioritize effectively.
  • Proficiency in Microsoft Office products.
  • Licensed Practical Nurse license in Tennessee, Mississippi, or Arkansas.
