Overview

Mae is looking for a Care Coordinator who is passionate about maternal health equity to support Medicaid payer pilots and member level engagement to address the growing disparity in Black maternal health outcomes.

In Short

  • Conduct needs assessments and screeners including various parameters.
  • Utilize influencing/motivational skills for member engagement.
  • Help members participate in healthcare decision-making.
  • Advocate for members' care needs and address gaps.
  • Manage care plans and referrals with payer teams.
  • Communicate with members via phone and email.
  • Promote appropriate use of benefits among members.
  • Identify and escalate quality of care issues.
  • Facilitate a team approach to member care.
  • Identify high-risk factors impacting member outcomes.

Requirements

  • Highly organized and able to manage multiple workstreams.
  • 3-5 years experience in social work, nursing, or healthcare.
  • Knowledge of referral coordination to resources.
  • Experience in relationship management.
  • Strong communication and presentation skills.
  • Birthwork/Doula experience is a plus.
  • Bilingual (English & Spanish).

Benefits

  • This is a contract (1099) position.
  • 40 hrs/weekly, Monday – Friday with flexible hours.
  • Occasional evening hours may be required.
