• Assess individuals in hospitals for care packages or care home placements under the Care Act.
• Conduct Mental Capacity Assessments for care and support.

Requirements

• Social Work England registration.
• Eligibility to work in the UK.
• Full UK driving license.
• Post-qualified experience.
• Degree level or equivalent qualification in Social Work.
