Overview

A skilled Social Worker is needed to join the Preparation for Adulthood Team, focusing on supporting young people aged 14-25 as they transition into adulthood.

In Short

  • Conduct comprehensive Care Act assessments for young people aged 14-25.
  • Utilize a strengths-based approach to support and empower individuals.
  • Collaborate with multidisciplinary teams to develop and implement care plans.
  • Provide support and guidance to young people and their families.
  • Maintain accurate and up-to-date case records.
  • Communicate effectively with clients, families, and other professionals.
  • Work one day a week at County Hall, with the remainder on a hybrid basis.
  • Manage a manageable caseload.
  • Enjoy excellent flexible work opportunities.
  • Receive an excellent rate of up to £19.35 per hour.

Requirements

  • Social Work England registration.
  • Eligibility to work in the UK.
  • Qualification in Social Work at degree level.
  • Post-qualified experience.
  • Experience in conducting Care Act assessments.
  • Strong analytical skills and critical thinking.
  • Experience working with children or adults in transition.
  • Adept at building relationships and effective communication.
  • Commitment to continuous professional development.
