Liquid Personnel is currently recruiting for experienced and qualified Social Worker to join a busy Hospital Discharge Team based in Lewisham.

Requirements

• Knowledge on the CHC process and discharge to assess pathways
• Skilled report writing and manage the recording and administration around assessment and planning.
• Identify and carry out Mental Capacity Assessments and chair best interest meetings.
