• Conduct thorough assessments under the Care Act to determine patients’ needs.

Requirements

• Minimum of 2 years of experience in relevant teams.
• Experience undertaking assessments and ensuring timely discharges.
• Registered with Social Work England.
• Eligible to work in the UK.
• Full UK driving licence required.
• Post-qualified experience and a degree level qualification in social work.
