An exciting job role has recently become available at Wrexham. They are looking for a new member for their Adult's Single Point of Access Team.

In Short

• First point of contact for referrals to Adult Social Care.
• Handle enquiries from various professionals and individuals.
• Assess and determine appropriate pathways for referrals.
• Conduct initial assessments and engage in What Matters conversations.
• Provide information, advice, or signposting to other services.
• Work alongside a multi-skilled team.
• Supported by an assistant Team Manager.
• Opportunity for remote work after a probationary period.
• Immediate start with initial 3-month contract available.

Requirements

• Social Work England registration.
• Eligible to work in the UK.
• Hold a full UK licence.
• Post-qualified experience.
• Degree level or equivalent in Social Work.
• Consolidation Award / Post Qualifying Award.
