Requirements

• Conduct comprehensive social work assessments for adults with learning disabilities.
• Develop and implement personalized care plans in collaboration with individuals, their families, and other professionals.
• Advocate for the rights and needs of individuals with learning disabilities.
• Provide support and guidance to individuals and their families on accessing services and resources.
• Work within a multi-disciplinary team to ensure holistic care and support.
