Are you an experienced adult social worker looking for a new opportunity? Westminster Hospital Discharge team are in need of a new social worker to replace someone who is going on maternity leave. They will offer a competitive rate and hybrid working.

In Short

• Undertaking section 2s and 5s
• Facilitating hospital discharge
• Carrying out assessments in different wards
• Managing a busy caseload within an acute hospital setting
• Working closely with health colleagues
• Ensuring adults in hospital are discharged in a timely manner
• Attending multi-disciplinary meetings
• Conducting mental capacity assessments
• Advising patients and families on eligibility criteria
• Redirecting patients to other agencies

Requirements

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK license
• Post-qualified experience
• Degree level or equivalent in Social Work
