An exciting job role has recently become available at the Somerset. They are looking for a new member for their Intermediate Care Team.

In Short

• You will manage a caseload of varying complexity.
• Receive and respond to referrals from various sources.
• Implement Community Care and an integrated care program approach.
• Conduct risk assessments to ensure safety and protection.
• Conduct statutory reviews and comply with legislation.
• Assess client needs in consultation with them and other professionals.
• Engage with individuals who refuse services but are at risk.
• Navigate sensitive negotiations during assessments.
• Effectively manage crises and provide quick resolutions.
• Work in a supportive team with a diverse caseload.

Requirements

• Social Work England registration.
• Eligibility to work in the UK.
• Full UK driving licence.
• Post-qualified experience.
• A degree or equivalent qualification in Social Work.
