Freelance Case Manager for Medical Rehabilitation - Remote

Overview

We are looking for social workers, educators, psychologists, or human scientists as case managers on a freelance basis.

In Short

• Optimizing the clarification of medical care and positive performance profile of rehabilitants.
• Job counseling and placement for rehabilitants: profiling, application training, and job acquisition in individual pedagogical support with corresponding documentation.
• Person-centered “pilot” function for systematic management of care.
• Cooperation in the network with service providers and support organizations.

Requirements

• Completed degree in social work, pedagogy, or psychology with at least 2 years of experience in the field of professional orientation.
• Completed psychology degree requires proof of at least 40 teaching units of social law knowledge.
• Training in health and social services or rehabilitative further education with at least 5 years of experience in professional orientation.
• Proof of methodological and social law competencies, customer orientation, motivation, empathy, and persuasiveness.
