Medical Case Manager - Remote

Overview

BlueCross BlueShield of Tennessee (BCBST) is seeking a skilled and compassionate Registered Nurse with a strong background in case management to join our fast-paced, high-performing team. This role focuses on supporting members with acute care needs and/or chronic health conditions, ensuring they receive timely, coordinated, and effective care.

In Short

• Support members with acute care needs and chronic health conditions.
• Engage and connect with members over the phone.
• Ensure timely and effective care delivery.
• Manage complex patient needs with empathy and clear communication.
• Perform essential case management activities.
• Maintain digital literacy and communicate via digital channels.
• Available for occasional late shift rotation.
• Requires a commitment of 18 months before applying for other internal positions.
• Must have or obtain Certified Case Manager credential.
• Work is fully remote.

Requirements

• Active Registered Nurse (RN) license in Tennessee or participating state.
• 3 years of clinical experience required.
• 5 years of experience in the healthcare industry.
• Excellent oral and written communication skills.
• Basic PC skills required (Microsoft Office and E-Mail).
• Must adhere to driver safety program guidelines if driving is required.
• Ability to manage complex cases effectively.
• Ability to work independently and collaboratively in a team environment.
• Strong interpersonal abilities.
• Proficient in Microsoft Office suite.
