Newborn/Pediatric Nurse Care Manager - Remote

Overview

We are seeking a Newborn/Pediatric Nurse Care Manager who is dedicated to providing holistic care to members throughout their healthcare journey. This role involves guiding members through complex medical and behavioral health situations while collaborating with a diverse clinical team.

In Short

• Deliver coordinated, patient-centered care management via telephone and video.
• Create impactful care plans with members and the care team.
• Assist members in navigating complex medical conditions and benefits.
• Partner with local providers for coordinated care.
• Provide compassionate follow-up care and support.
• Coordinate resources to address both clinical and social problems.
• Work 9 AM to 6 PM PST, Monday to Friday.

Requirements

• Bachelor of Science in Nursing (BSN).
• Current CCM Certification or eligibility within 6 months of hire.
• 5+ years of nursing experience.
• 2+ years in care management.
• Must reside in a compact NLC state.
• Active Compact RN license in good standing.
• Willingness to maintain licensure in multiple states.
• Experience with various age populations and medical conditions.
• Strong computer/medical record system skills.
• Empathetic and responsive communication skills.
