Overview

We are seeking a passionate Newborn/Pediatric Nurse Care Manager to provide holistic care and support to members through their healthcare journey, ensuring their needs are met with industry-leading interventions.

In Short

• Deliver coordinated, patient-centered virtual Care Management via telephone and video.
• Create impactful care plans with members and the care team.
• Navigate complex medical conditions and healthcare systems.
• Provide compassionate longitudinal follow-up care.
• Assist during acute healthcare episodes.
• Coordinate resources to address clinical and social problems.
• Work Monday-Friday, 9am-6pm PST.

Requirements

• Bachelor of Science in Nursing (BSN).
• Must reside in a compact NLC state.
• Active Compact RN license in good standing.
• Active California Nursing License.
• Current CCM Certification or eligibility to sit for the exam.
• 5+ years of Nursing experience.
• 2+ years in Newborn/Pediatric setting.
• Comfortable discussing various medical conditions.
• Strong competence in using medical record systems.
• Empathetic and responsive to patient needs.
