Newborn/Pediatric Nurse Care Manager - Remote

Overview

We're looking for a Newborn/Pediatric Nurse Care Manager for our Care and Case Management team, who is passionate about caring for members holistically through their healthcare journey and ensuring needs are met with industry-leading interventions.

In Short

• Deliver coordinated, patient-centered virtual Care Management by telephone and video.
• Create impactful care plans together with members and our diverse care team.
• Help members navigate complex medical conditions and the healthcare system.
• Partner with members' local providers for coordinated care.
• Provide compassionate, longitudinal follow-up care.
• Assist throughout acute healthcare episodes.
• Coordinate necessary resources addressing clinical or social problems.

Requirements

• Bachelor of Science in Nursing (BSN).
• Must reside in a compact NLC state.
• Active Compact RN license in good standing.
• Willingness to become licensed in multiple states.
• Current CCM Certification or eligibility for CCM.
• 5+ years Nursing experience.
• 2+ years in Newborn/Pediatric setting.
• Comfortable discussing several medical conditions.
• Empathetic and responsive.
• Strong competence in computer/medical record systems.
