Overview

We are seeking a passionate Newborn/Pediatric Nurse Care Manager to join our Care and Case Management team, responsible for guiding members through their healthcare journey with a focus on holistic care.

In Short

• Deliver coordinated, patient-centered virtual care management by telephone and video.
• Create impactful care plans with members and the care team.
• Help members navigate complex medical conditions and the healthcare system.
• Partner with local providers for coordinated care.
• Provide compassionate follow-up care and build supportive relationships.
• Assist during acute healthcare episodes with coordinated case management.
• Coordinate resources to address clinical and social problems.

Requirements

• Bachelor of Science in Nursing (BSN).
• Must reside in a compact NLC state.
• Active Compact RN license in good standing.
• Willingness to become licensed in multiple states.
• Current CCM Certification or eligibility to sit for the exam.
• 5+ years nursing experience.
• 2+ years in a Newborn/Pediatric setting.
• Ability to work Monday-Friday, 9am-6pm PST.
• Empathetic with strong communication skills.
• Competence in using multiple computer/medical record systems.
