Case Manager – Contingency Management - Remote

Overview

A Case Manager is needed to support young adults in a Contingency Management program aimed at changing their relationship with stimulant use, working in a flexible, remote environment.

In Short

• Manage a caseload of 20–30 individuals.
• Conduct virtual check-ins and crisis support.
• Complete intakes and assessments for new participants.
• Track and document behavioral outcomes and goals.
• Coordinate urinalysis testing results.
• Assist with distribution of program incentives.
• Maintain documentation in the electronic health record.
• Connect individuals to community resources.
• Support outreach efforts for program access.
• Ensure compliance with HIPAA and reporting requirements.

Requirements

• Bachelor’s degree in Social Work, Psychology, or related field.
• Minimum of 2 years of case management experience.
• Experience with trauma-informed care.
• Strong documentation and communication skills.
• Ability to engage young adults in a virtual environment.
• Complete a Level II background screening.
• Valid driver's license and auto insurance.
