Senior Community Management Professional - Remote

Humana Healthy Horizons in Virginia is seeking a Senior Community Management Professional who will be responsible for directing community engagement practices and linking our membership to services critical to the improvement of health outcomes.

In Short

• Direct community engagement practices
• Facilitate and plan Member and Community Advisory Panels
• Collaborate with medical and service providers
• Engage in community boards and committees
• Drive outcomes aligned with organizational goals
• Identify service gaps with strategic partners
• Build need-based partnerships with social service providers
• Participate in outreach programs
• Work remotely with travel throughout Virginia
• Must reside in Virginia

Requirements

• Bachelor’s degree
• 2+ years in public health or community engagement
• Experience in project management and resource mobilization
• Understanding of Medicaid access barriers
• Strong facilitation and conflict management skills
• Ability to manage multiple projects
• Bilingual preferred (English/Spanish, Arabic, etc.)
• Valid driver's license and reliable transportation
• TB screening required
• Must pass language proficiency assessment
