Overview

The Social Determinants of Health (SDOH) Coordinator plays a crucial role in connecting enrollees with community-based resources to address their health-related social needs.

In Short

• Responsible for finding community-based support for enrollees.
• Support Case Managers and Community Health Workers.
• Maintain an updated resource list for the region.
• Conduct outreach and assessments for enrollees.

Requirements

• 2+ years of experience with community resources.
• Case Management experience.
• Intermediate knowledge of Microsoft Office.
• Exceptional communication and interpersonal skills.
• Ability to multi-task in a fast-paced environment.
• Strong understanding of cultural diversity.
• Strong written communication skills.
• Bachelor’s Degree in Social Work or related field preferred.
• Fluency in Spanish, French Creole, or Vietnamese is a plus.
