Enhanced Community Care Management (ECCM) is seeking a social worker to join its team in providing palliative care services, focusing on psychosocial assessments and interventions for patients with complex needs.

Requirements

• Basic computer skills, including Microsoft Office.

• Travel required to various work sites.
