Overview

The Patient Care Plan Coordinator is responsible for supporting the clinical strategy by developing and implementing patient care plans for frequent callers and patients with complex needs.

In Short

  • Support the delivery of clinical strategy through new patient care plans.
  • Mentor clinical champions in writing and implementing patient care plans.
  • Identify frequent callers who may benefit from a care plan.
  • Ensure existing care plans are safe and current.
  • Work with healthcare professionals and families to develop effective care plans.
  • Escalate issues with internal stakeholders as necessary.
  • Promote the vision of patient care plans to internal stakeholders.
  • Contribute to audits and data reviews for ongoing improvement.

Requirements

  • Registered health professional with current practicing certificate.
  • Experience in building relationships with health stakeholders.
  • Extensive experience in implementing patient care plans.
  • Experience handling confidential patient information.
  • Knowledge of challenges in health and social care.
  • Strong written and oral communication skills.
