• The Nurse Practitioner delivers annual risk assessment in a residential setting or telehealth.
• Conduct comprehensive in-home health risk assessments.
• Takes history, examines, determines diagnoses.
• Provides written documentation of patient visit.
• Communicates with patients and other healthcare providers.
• Maintains patient confidentiality.
• Participates in professional development activities.
• May work with various healthcare team members.

Requirements

• Active/unrestricted nurse practitioner license.
• Board certification from ANCC, AANP, or NCCPA.
• Active CPR Certification.
• Current enrollment in Medicare/Medicaid.
• Outstanding EHR skills.
• Geriatric training/experience preferred.
