In Short

• This is a full-time salaried role with a Monday-Friday schedule, 8-5pm EST, with a rotating late day once a week to support late staff until 7pm.
• Lead the Care Navigator team responsible for intake processes.
• Ensure the intake pipeline runs smoothly and referrals are managed appropriately.
• Coach and evaluate team members to achieve performance standards.
• Monitor performance against key metrics.
• Collaborate with Clinical Outreach teams to improve referral accuracy.
• Conduct performance reviews and implement improvement plans.
• Ensure compliance with HIPAA and internal quality standards.

Requirements

• Bachelor’s degree required; advanced degree preferred.
• 5+ years of intake, scheduling, or referral management experience; 2+ years in leadership preferred.
• Proven ability to manage high-volume intake operations.
• Strong data analysis skills.
• Excellent written/verbal communication and relationship management skills.
