The Field Care Manager, Behavioral Health 2 plays a crucial role in assessing and evaluating the needs of members to maintain optimal wellness, guiding them towards appropriate resources for their care.

In Short

• Assess and evaluate member needs for optimal wellness.
• Conduct in-home visits and telephonic assessments.
• Manage a caseload of members effectively.
• Develop and coordinate care plans.
• Address barriers to health care for members.
• Collaborate with interdisciplinary care teams.
• Utilize electronic medical records and software applications.
• Travel 75% of the time to member residences.
• Must have a valid driver's license and car insurance.
• Work full-time (40 hours minimum) Monday to Friday.

Requirements

• Licensed Bachelor's prepared Social Worker.
• Minimum of 1 year in-home experience.
• Experience with adult population and disease management.
• Exceptional communication and interpersonal skills.
• Ability to work independently.
• Intermediate to advanced computer skills.
• Access to a home office with high-speed internet.
• Valid driver's license and proof of liability insurance.
• Knowledge of community health and social service agencies.
• Bilingual skills are a plus.
