Overview

The Nurse Care Manager will provide compassionate care to patients with kidney disease, ensuring they receive person-centered, holistic care while managing their healthcare journey.

In Short

• Work closely with the Senior Director of the Evergreen Compassionate Care Program.
• Manage healthcare journeys based on patients' unique needs and goals.
• Collaborate with a team of healthcare professionals.
• Perform assessments and advance care planning conversations.
• Coordinate care transitions and referrals.
• Educate patients and families about care options.
• Engage with social determinants of health.
• Maintain accurate patient records.

Requirements

• Current RN License required, BSN preferred.
• 1+ years of utilization management experience preferred.
• 3+ years of clinical practice in ICU, hospice, or palliative care.
• Bilingual (Spanish) preferred.
• Experience with goals of care discussions required.
• Intermediate skills with MS Office Suite.
• Able to work effectively in a primarily remote environment.
• Accepts rapid changes in a startup environment.
