In Short

• Provide client support focusing on problem-solving, skills training, behavior modeling, optimism, and encouragement.
• Deliver interventions in the office, virtually, in the community, and at clients' residences based on client preferences, needs, and clinical direction.
• Enroll eligible individuals in care management programs as needed.
• Assist the team and clients in ongoing assessment of symptoms and responses to interventions.
• Contribute to the development of treatment plans as part of the client's treatment team.
• Ensure treatment goals are culturally competent, strength-based, realistic, attainable, and promote recovery.
• Ensure each individual is actively involved in their treatment planning process, and their preferences are incorporated.
• Coordinate and ensure timely face-to-face contact with enrollees in the community following inpatient discharge.

Requirements

• Bachelor’s degree in Psychology, Social Work or related field.
• 3 year experience working with adults with mental illness and/or substance abuse.
• Fluent in Spanish and English.
• Valid Massachusetts Driver's License.
