Care Manager - Community Behavioral Health Center - Remote

Overview

As a Care Manager at Eliot, you will be part of a compassionate team dedicated to providing in-home support and services to clients in the Community Behavioral Health Center.

In Short

• Provide client support focusing on problem-solving and skills training.
• Deliver interventions in various settings including clients' homes and the community.
• Enroll individuals in care management programs.
• Assist in ongoing assessment of symptoms and responses.
• Contribute to the development of treatment plans.
• Ensure treatment goals are culturally competent and promote recovery.
• Involve individuals in their treatment planning process.
• Coordinate timely face-to-face contact with clients post-discharge.

Requirements

• Bachelor’s degree in Psychology, Social Work, or related field.
• 3 years of experience with adults with mental illness or substance abuse.
• Valid Massachusetts Driver's License.
• Fluency in Spanish is a plus.
