Overview

We are seeking an experienced and compassionate Nurse Practitioner (NP) to join our team providing holistic care to kidney patients—specifically kidney transplant recipients in Smyrna, GA. This full-time hybrid-remote role includes both in-person and virtual responsibilities, combining the best of advanced practice nursing and nephrology care coordination.

In Short

  • Full-time hybrid-remote role with in-person and virtual responsibilities.
  • Deliver high-quality care to kidney transplant recipients.
  • Perform Evaluation & Management (E&M) visits.
  • Review labs and manage medications and referrals.
  • Maintain comprehensive electronic health records (EHR).
  • Collaborate with supervising nephrologists.
  • Flexible schedule with no nights or weekends.
  • Must travel within assigned geography to clinics.
  • Requires current NP license and minimum 2 years of experience.
  • Spanish-speaking ability preferred in certain markets.

Requirements

  • Current unrestricted NP license to practice in the state.
  • Minimum 2 years of NP experience in Family Practice, Internal Medicine, or General Medicine.
  • DEA license (or eligible within 90 days of hire).
  • Current BLS/CPR certification.
  • Proficient in Epic or similar EHR system.
  • Intermediate computer skills with Microsoft Office.
  • Home office setup with high-speed internet.
  • Experience in nephrology or transplant management preferred.
  • Background in managing chronic diseases preferred.
  • ANNA Certification strongly preferred.
